▶Male menopausal symptoms
While it is widely known that women experience menopausal symptoms, are you aware that men can also experience a condition known as male menopause?
As men age, the secretion of the male hormone (testosterone) declines, leading to symptoms such as autonomic nervous system disorders and LOH syndrome (late-onset hypogonadism).
These symptoms can vary greatly and are often misdiagnosed as unexplained fatigue or depression. However, they may actually be caused by the decrease in male hormones.

Anxiety, irritability
Insomnia
Depression
Decreased appetite, decreased memory
Hot flashes, dizziness

Loss of morning erections

If these symptoms apply to you, it is possible that they are caused by a decrease in male hormones.
To diagnose the condition, a blood test is conducted to measure testosterone levels, and if they are found to be below the internationally established reference values, testosterone therapy is considered.
Testosterone therapy for male menopause is an approved treatment by the Japanese government.
There are many university hospitals and clinics that specialize in treating male menopause or offer men's health care in Japan.

▶Administration method

▶Side effects
①Gonadal Dysfunction (Azoospermia)
Prolonged testosterone supplementation may lead to a decline in testicular function and can potentially cause conditions such as azoospermia (absence of sperm). When testosterone is externally supplemented, the endogenous testosterone produced is no longer secreted, resulting in a decrease in gonadal function. If the development of azoospermia is believed to have a significant impact on your life, supplementation with hormone hCG, which stimulates the testes to produce endogenous testosterone, can be considered. ②Impact on the Prostate
Testosterone has been known to promote prostate enlargement. Currently, testosterone therapy is not recommended for patients with moderate to severe benign prostatic hyperplasia (enlarged prostate). Please discuss this matter during the consultation.
③Influence on Hair Loss or Thinning
Some individuals may be concerned that injecting testosterone will lead to baldness. In reality, it is not testosterone itself but a substance called DHT (dihydrotestosterone), which is formed when testosterone binds with an enzyme called 5-alpha reductase, that is implicated in hair loss. The amount of testosterone in the body is not a significant factor, but rather the high activity of the enzyme 5-alpha reductase, resulting in increased production of DHT, is a major cause. While excessively high levels of testosterone in the bloodstream can have an impact, maintaining an appropriate concentration is generally not considered to significantly affect hair loss. However, if you have concerns, it is possible to take a hair loss prevention medication (5-alpha reductase inhibitor) in conjunction with testosterone therapy. Please consult for more information.
④Gynecomastia (Enlarged Breasts)
Gynecomastia is a condition characterized by the swelling of the breasts, resembling female breasts. Both males and females secrete female hormones, and when the proportion of female hormones to male hormones increases in males, breast enlargement can occur. Testosterone can actually be converted to estrogen, a type of female hormone, by an enzyme called aromatase, and long-term use of testosterone can lead to the development of gynecomastia. Generally, if testosterone levels are maintained within a moderate range, it should not cause any issues. However, an aromatase inhibitor medication is used as a preventive measure.
⑤Liver Disfunction
Continued administration of testosterone injections can potentially affect liver function. If liver disfunction is detected through blood tests, the medication plan will be reassessed and additional medications may be prescribed to address the situation.
